Bookings

For Quotes or Bookings:

CEFA assesses each request for first aid services individually, and we really need a completed booking form to even provide a quote. Certainly there is no obligation when requesting a quote, simply check the "quote" or "booking request" boxes at the top of the form.

To request either, down load one of the forms below, fill it out, and return it. The pdf is probably the safest, but you will have to print it, fill it out, and either scan it and email the scan back, or post it back. The two spreadsheet formats should work, but the formatting is "temperamental". Earlier Excels are probably best with the xls sheet, but later versions of Excel should open, and may well work better, with the open document format.

The spreadsheets think they have macros because they use form "checkboxes", but there is no need to "enable macros" when you open the sheet if you have the choice.

We are working on a fully online version of the booking sheet, and hope to have it usable "real soon now".
